Student Supply Lists 2021-2022

Student Registration for the 2021-2022 School Year

We are proud to offer a free full day PreK program for resident three and four year old students. The PreK program follows the regular school schedule and calendar.

After providing the necessary information, you will receive an email from OnCourse Notifications to start the process. You will also need to schedule an appointment with District Secretary Terry Harris to provide the required paperwork as outlined below. Registration Paperwork

1. Your child’s original birth certificate with the state seal
Pre-Kindergarten- 3 years of age on or before October 1, 2021
Kindergarten- 5 years of age on or before October 1, 2021

2. Proof of Immunizations- Please bring your child’s most current health records. Your child must also have a physical examination completed by your family doctor. You may use THIS FORM or your physician’s form if the information is the same. Please make sure that your child's lead testing results are included on the health records. 3. Proof of Residency- A valid form of identification showing a National Park home address along with two of the following documents:
Current tax bill, utility bill or cell phone bill
Mortgage settlement paperwork (showing date of occupancy) or Lease Agreement

4. If your child is a transfer student within the state of New Jersey, you will need to provide a Transfer Card from the previous school district and complete a Records Release form.

Note: Please provide Guardianship/Custody Papers and/or a Landlord Affidavit if applicable.

HIB Self Assessment GradeBeginning with the 2011-2012 school year, our school district has conscientiously implemented the requirements found in the Anti-Bullying Bill of Rights Act by implementing a number of strategies to prevent harassment, intimidation and bullying. While completing the Self-Assessment, we learned that our school district has demonstrated strengths in providing a wide variety of opportunities for our students to engage in positive learning experiences focused on character building. We continue to make improvements as we increase staff training opportunities and fine tune our character education program. If you have any questions regarding HIB, please contact Mrs. Bittner or Ms. O’Donnell.
